The total fee that includes the application for examination and the clinical/practicum examination is $800.00. An
application fee of $50.00 must be submitted along with a written request for an
application (this is a non refundable fee). To be eligible to take the examination the completed
application along with the remainder of the fee in the amount of $750.00 must be received in the Board office 60 days prior to the scheduled starting date of
the examination being applied for (see application deadlines on enclosed schedule).
